Plan of Study

This form is reserved for declared Family and Consumer Science students with 24 or more hours earned. Please read and follow the directions and suggested format.

Who should complete this form and when?

  • Declared FCS students with 24 hours or more earned hours are advised in the FCS Advisement Office and are required to complete this entire form, print it and meet with the FCS advisor to review it. (See directions below.)
  • Students must have the form completed and reviewed by the advisor by the end of their sophomore year.
  • Transfer students must have the form completed and reviewed by the end of their first semester at Illinois State University.
  • You will be expected to have your copy of your plan with you to for all meetings with the advisor.
  • It is recommended that you refer to your plan when corresponding via email with the advisor regarding scheduling issues.

Directions:

  • Pick up an Academic Progress Summary, Moulton Hall 107, to see exactly what classes you need, and to be sure of the courses that you have taken. Take your ID with you!
  • It is recommended that you take time to develop your plan of study on paper first and then complete this form--refer to your sequence sheet to be sure that you take classes in the suggested order.
  • Be sure that you allow yourself an adequate amount of time to complete this form correctly and completely.
  • Use the TAB key to navigate throughout the form.
  • For each semester, type in what you have taken, or what you plan to take (again, see your  Academic Progress Summary for a complete listing).  Plan all of your semesters through graduation!
  • For students following ISU's General Education Program (Inner, Middle, Outer Core) please indicate next to the course what category it is satisfying (MC-UST for example). 
  • Things to keep in mind while you develop your plan:
    -Prerequisites (see your catalog)
    -Course offerings (Fall and Spring only, see your catalog, sequence sheet or the FCS Web site curriculum page for your program)

Recommended format:  
(Department abbreviation, course number, number of credit hours, General Education category if applicable, minor if applicable) Example: 

  • FCS 101 (3)
  • HIS 111 (*MC-UST, 3)   Or if you are a transfer student following IAI indicate which category the course is satisfying.
  • ECO 105 (Minor, 4) Indicate courses counting towards your minor.

*MC-UST refers to ISU's general education program.  
Transfer students should indicate if they have an associates degree in the comments box at the end of this form OR indicate if they are following the Illinois Articulation Initiative program (IAI) and which courses are counting toward this program. Ask the advisor if you have questions concerning your situation jsfalco@ilstu.edu.
